Replace "Needs review" with Y/N

Since there no longer is a way to review "Fill in the blank" and Free text, then the "Needs review" column should just be a Yes/No state, if the quiz either have "Fill in the blank" or Free text.

That should also remove a fair amount of code and Mongo queries I suppose =)

To upload designs, you'll need to enable LFS and have an admin enable hashed storage. More information
